Nearly 10,000 First Visits Since 2012
The eight Trànsit units, the Catalan Health Institute's (ICS) specialised service for trans and non-binary people's health, have recorded 9,858 first visits between 2012, when the first unit opened, and 2025. The data was provided by the Department of Health in response to a parliamentary question from the PP and published on 11 October 2026. Over half of these initial consultations, 52% (5,118), were by people aged 15 to 24. Of all patients seen, 52.2% identified as women (5,142), 45.7% as men (4,507), and 2.1% had unspecified sex (209).
Sharp Rise in Surgical Procedures
Surgical interventions have risen continuously, from 16 in 2012 and 15 in 2015 to 139 in 2025, the highest annual total recorded. This represents a 36.3% increase from 2024, driven largely by a surge in mastectomies, which more than doubled from 25 to 54. Vaginoplasty accounted for 59% of the 722 surgeries performed overall, with 63 procedures carried out in 2024. Glottoplasty made up 11.5% (83 operations).
The Trànsit units provide initial assessment and, where appropriate, coordinate a personalised care pathway including surgical referrals.
Age Patterns in Care
While 52% of first visits were by 15–24-year-olds, the 25–34 age group accounted for 34.3% of all surgeries (248), ahead of those under 24 (31.4%, or 227). Nearly seven in ten operations were performed on people under 35. By procedure type: vaginoplasty was most common among under-24s (147), mastectomy among 25–34-year-olds (99), and glottoplasty among 45–54-year-olds (24).
Service Locations and Referrals
The eight Trànsit units operate through primary care centres in Badalona, Barcelona, Girona, Lleida, Manresa (at the CUAP Bages), Reus, Sabadell and Terrassa. The Barcelona unit coordinates referrals to three reference hospitals under the official surgical protocol: Hospital Clínic, Hospital Universitari de Bellvitge, and Hospital Universitari Trias i Pujol. First visits rose from 19 in 2012 to 1,059 in 2025, though this was down from a peak of 1,454 in 2021. The 2025 figure marked a 6.12% increase from 2024.
